    2024-2025 Student Organization Registration

    Student Organizations:

    Complete this form to register your organization for the 2023-2024 academic year.

    What you'll need to have ready:
    • Officer (must have at least a President, Vice President, Secretary, and Treasurer)
    • Member (must be at least 5 student members. This includes officers)
    • Advisor (must be full-time faculty or staff)
    • Constitution (download a sample at http://www.utpb.edu/life-at-utpb/student-life/student-organizations/rso-sample-constitution.pdf)

    In addition to completing this form, three organization officers and the advisor must also complete organization orientation. Once your form has been reviewed Student Life will contact you to schedule orientation.

    Organization Type
    There are two organization types and each organization must select one of these types from the list. Organizations who select "University-Sponsored" will have an extra step in the registration process. The organization and sponsoring department will have to complete a form agreeing to the sponsorship. That form will be sent directly to the organization after this initial registration form is received. 
    • ​Registered Student Organization 
      • Registered Student Organizations are those that are not a function of the University but seek to be recognized by the institution as a means to function on campus with access to institutional resources. Most organizations will be this type.
    • University-Sponsored Organization 
      • University-Sponsored Organizations are those that receive institutional support and/or serve as a function of a University department. Institutional support may be fiscal, an advisor who serves as part of their job description, etc. Examples include Student Government Association and the Campus Activities Board.
    Organization Category
    All organizations are further broken down into categories. In addition to selecting organization type, you also need to select categories according to how your organization identifies. This helps the Office of Student Life with reporting and helps potential members seek out your organization by purpose.

    Roster
    Organizations must have a minimum of five student members to be considered active. All organizations must have a President, Vice President, Secretary, and Treasurer. In the following sections we will ask you to provide the name and contact information for each individual member. 
    Organization Constitution
    All student organizations are required to upload a constitution. Constitutions must include a non-discrimination clause and a membership clause. 
    As a reminder, non-university sponsored organizations may NOT use the University's name as part of their organization name. However, you can use the University's name to indicate location. 

    Not acceptable: UT Permian Basin Gamer's Club 
    Acceptable: The Gamer's Club at UT Permian Basin
