Student Organizations:

Complete this form to register your organization for the 2023-2024 academic year.

What you'll need to have ready:

Officer (must have at least a President, Vice President, Secretary, and Treasurer)
Member (must be at least 5 student members. This includes officers)
Advisor (must be full-time faculty or staff)

In addition to completing this form, three organization officers and the advisor must also complete organization orientation. Once your form has been reviewed Student Life will contact you to schedule orientation.

Organization Type

There are two organization types and each organization must select one of these types from the list. Organizations who select "University-Sponsored" will have an extra step in the registration process. The organization and sponsoring department will have to complete a form agreeing to the sponsorship. That form will be sent directly to the organization after this initial registration form is received.

Registered Student Organization
- Registered Student Organizations are those that are not a function of the University but seek to be recognized by the institution as a means to function on campus with access to institutional resources. Most organizations will be this type.
University-Sponsored Organization
- University-Sponsored Organizations are those that receive institutional support and/or serve as a function of a University department. Institutional support may be fiscal, an advisor who serves as part of their job description, etc. Examples include Student Government Association and the Campus Activities Board.

Organization Constitution

All student organizations are required to upload a constitution. Constitutions must include a non-discrimination clause and a membership clause.
As a reminder, non-university sponsored organizations may NOT use the University's name as part of their organization name. However, you can use the University's name to indicate location.

Policy Agreements

All organizations must agree with the following statements in order to be a registered organization on campus.

Anti-Discrimination Statement

This organization does not deny membership on the basis of race, ethnicity, religion, national origin, gender, age, disability, citizenship, veteran status, sexual orientation, gender identity or gender expression, except that a) an organization created primarily for religious purposes may restrict the right to vote and hold office to persons who subscribe to the organization’s statement of faith.

Anti-Hazing Statement

The members of this organization will not participate in any Hazing activity. Hazing in state educational institutions is prohibited by state law (Texas Education Code section 51.936 and sections 37.151-37.157) and the UT Permian Basin Student Code of Conduct (Section 1.410).

INDIVIDUALS A person commits an offense if the person: Engages in hazing; Solicits, encourages, directs, aids or attempts to aid another engaging in hazing; Recklessly permits hazing to occur; or Has firsthand knowledge of the planning of a specific hazing incident involving a student in an educational institution, or has firsthand knowledge that a specific hazing incident has occurred, and knowingly fails to report that knowledge in writing to the dean of students or other appropriate official of the institution.

ORGANIZATIONS: An organization commits an offense if the organization condones or encourages hazing or if an officer or any combination of members, pledges, or alumni of the organization commits or assists in the commission of hazing.
